Procedure Overview 

for Reporting Theft of Equipment and/or Cargo

 

The State of Florida in cooperation with the Florida Trucking Association and the Florida Joint Multi-Jurisdictional Task force has established a reporting system for stolen Equipment and Cargo through their "Fax-Alert" Program

We endorse the program and contained herein is our procedure for reporting the theft of Equipment and or cargo through the use of the "Fax-Alert" Program.

FIRST, report all thefts to Steve Brown or Ida Nelson in our safety department  by calling 800-444-4168 and fax all copies of interchanges, bills of lading, and/or delivery instructions plus a copy of the "Fax Alert"  report to Steve or Ida at 704-283-7122. A preliminary copy should accompany the initial initial and a final copy after the Investigating Officer has completed their report.  Be sure that you get the Officers report number and the location of his office where the report will be filed.  This information should accompany the final report. 

SECOND, notify your local Law Enforcement Agency of the theft to initiate the local investigation process.

Included with your Theft Reporting procedures is a blank copy of the "Fax Alert" reporting form and the State of Florida's "Fax Alert Procedure".  In addition we have included a "Sample" form for your use when having to complete a form when a theft occurs.

Timely reporting of thefts is essential and should be your highest priority when you discover that a theft has occurred.

Once it is received, the Florida BOI will fax a Theft Recovery Form to the telephone number listed on the "Fax Alert" form. As soon as you receive this form, fax a copy of it to the Safety Department in Monroe.  

 All interchanges should include the chassis information including the license number and the state where licensed.  If this information is not on the interchange, it should be added by the first driver who picks the container and/or chassis at the port or Shipping/Rail Station.

Finally, more detailed instructions are contained in a packet sent to each of the Agencies.  Refer to this packet and make sure that a copy of the instructions and and several copies of the "Fax Alert" form are in every Tractor that handles Freight for Titan Transport, LLC.

The amounts for a Dry Container should be $10,000, and  for a refer $40,000.

Amounts for the cargo may not be readily available.  If you can estimate do so but list your numbers as an estimate only.

List company in all instances as Titan Transport LLC. 

When Completed, fax the form  to the number on the bottom.  The Database is maintained by an Florida Highway Patrol independent agency in Florida but is shared with the states of North and South Carolina, Virginia and Georgia within Minutes of receipt. 

A copy will be sent to all DOT weight stations, Agricultural Inspection Stations and all other agencies who have requested to be added to the notification system.  The system runs 24 hours a day, seven days a week.
